Client Agreement
As the client you agree that it is your choice to receive massage therapy, bodywork or aesthetics, & that it is your decision to partake in our classes & workshops. You acknowledge that you are aware of the benefits and risks of ALL of our services (massage, aesthetics, polarity, reiki etc.) & our classes/workshops (yoga, meditation, sound healing, ceremonies etc.) and give full consent for massage or physical touch during any of these modalities from ANY of our therapists & instructors. As the client, you understand that there is no implied or stated guarantee of success or effectiveness of individual techniques or series of appointments.
You acknowledge that massage therapy, aesthetics, bodywork, yoga, meditation etc. are not substitutions for medical care, medical examination or diagnosis & you have stated all medical conditions that you're aware of and will inform your practitioner of any changes in health status. The health waiver below MUST be filled out before any sessions, classes or hands-on workshops.
Payments
As the client, you are responsible for all charges of any services you've booked at Fisher Wellness. We do not offer refunds unless your session/class/event was cancelled by Fisher Wellness & we do not offer refunds on deposits unless your session was cancelled Fisher Wellness. If you do not cancel within 24hrs of your session you will be charged in full.
